Welcome to the Publix Customer Voice Survey Thank you for participating in the Publix Customer Satisfaction Survey at www.publixsurvey.com (the “Survey”). This Survey is designed to collect valuable feedback from customers to help Publix Super Markets, Inc. (“Publix”) improve its services, products, and overall shopping experience. By completing the Survey, you may enter the Publix Customer Voice Survey Sweepstakes (the “Sweepstakes”) for a chance to win a $1,000 Publix gift card.

Acceptance of Terms

By accessing or participating in the Survey and/or Sweepstakes, you agree to be bound by these Terms & Conditions, the Official Rules of the Sweepstakes, and all applicable laws and regulations. If you do not agree with any part of these terms, you must not participate.

Changes to Terms

Publix reserves the right to modify these Terms & Conditions or the Official Rules at any time without prior notice. Changes will be effective immediately upon posting on the official Survey page or related Publix websites. Your continued participation constitutes acceptance of those changes. It is your responsibility to review these terms periodically.

Eligibility

  • No Purchase Necessary to enter or win.
  • Open only to legal residents of the United States in the states where Publix operates (typically Florida, Georgia, Alabama, North Carolina, South Carolina, Tennessee, Virginia, and others as specified in current Official Rules).
  • Participants must be at or above the age of majority in their state of residence (generally 18+).
  • Employees of Publix, its affiliates, administrators (e.g., Bellomy Research, Inc.), and their immediate family members or household members are not eligible.
  • Void where prohibited by law.

How to Enter

  1. Online Entry (with Survey Completion): Visit www.publixsurvey.com, enter the required details from your recent Publix receipt (store number, survey code, timestamp), complete the Survey honestly, and submit your contact information for Sweepstakes entry.
  2. Mail-In Entry (No Purchase or Survey Required): Hand-print your name, email address, phone number, and date of birth on paper or an index card and mail to the address provided in the current Official Rules (e.g., Publix Customer Voice Survey Sweepstakes [Current Period] Entry, Attn: Marketing/Research, 3300 Publix Corporate Parkway, Lakeland, FL 33811).
  • Limit: One entry per survey code or mail-in per period, subject to Official Rules.
  • Entries must be received within the promotional period (e.g., quarterly or as specified).

Prizes and Odds

  • Typically, one or more $1,000 Publix gift cards per drawing period.
  • Prizes are non-transferable, non-redeemable for cash, and subject to gift card terms.
  • Odds of winning depend on the number of eligible entries received.

  • Only one prize per household per period, as per Official Rules.

User Conduct and Privacy

  • Provide honest and accurate feedback; false information may disqualify you.

Governing Law

These Terms & Conditions are governed by the laws of the State of Florida, without regard to conflict of laws. Disputes will be resolved in courts located in Florida.

Indemnification

You agree to indemnify and hold harmless Publix, its affiliates, and administrators from any claims arising from your participation or violation of these terms.

Termination

Publix reserves the right to cancel, suspend, or modify the Survey/Sweepstakes at any time.

Entire Agreement

These Terms & Conditions, along with the Official Rules (available at the Survey page or via request), constitute the full agreement.
